Aged Care | Support at Home

From 1 November 2025, the Australian Government’s Support at Home program replaces Home Care Packages, bringing together in-home aged care services to help older people live independently at home for longer.

All Support at Home services are delivered under a consumer-directed care approach, where you decide the level of involvement you want to have in managing your support. You will have more control over the types of care and services you access and the delivery of those services, including who delivers the services and when. You will be encouraged to identify goals, which could include gaining more independence or health goals. These will form the basis of your individual care plan.

The Consumer Directed Care approach gives consumers:

• More choice and flexibility
• Support to access relevant information and make informed decisions on the care that is best for them
• A partnership approach and better-quality participation
• Greater transparency

Coordinated Home Care subsidised by the Australian Government

Support at Home is a coordinated program of care and services to help you live independently in your own home for as long as you can, which can be especially helpful if you have complex needs that require support from Home Care providers.

Eligibility

Support at Home is open to seniors over 65 years of age, or over 50 years for Aboriginal or Torres Strait Islanders, who need:

• Ongoing services
• Multiple services, or one service at a high frequency
• High-level support, such as complex nursing care

If your needs are basic, such as requiring showering a few days a week or fortnightly cleaning, you might be eligible for the Commonwealth Home Support Program (which will become part of Support at Home no earlier than 1 July 2027).

Service Classifications

Depending on your needs, you will be approved for one of 8 ongoing service classifications. Each classification has a quarterly (3-monthly) budget based on your assessed needs. If your needs change, you can request a re-assessment.

If you were already receiving a Home Care Package, you will automatically move to Support at Home on 1 November 2025 with the same level of funding.

Services Available

Support at Home includes a wide range of services to help you stay living in your own home:

Clinical Care:

• Nursing services such as wound care, palliative care, or medication management
• Allied health services including physiotherapists, dieticians, podiatrists, and speech therapists
• Occupational therapy and continence care

Independence Support:

• Personal activities such as bathing, showering, toileting, dressing and undressing
• Assistance with mobility and communication
• Help with taking medications
• Transport to appointments and social activities
• Respite care to provide caregivers with temporary rest

Everyday Living:

• Meal preparation, nutrition and diet assistance
• Domestic assistance including general housework, cleaning, and washing clothes
• Social support to maintain an active social life
• Home maintenance including gardening and cleaning (when reasonable and necessary)

Additional Support Pathways

Restorative Care Pathway Up to 16 weeks of support to help you regain independence with daily tasks through multidisciplinary allied health services.

Assistive Technology and Home Modifications (AT-HM) Scheme Access to products, equipment (such as wheelchairs) and home modifications (such as shower rails) to meet your assessed needs, with funding up to $15,000 for each category.

End-of-Life Pathway Dedicated support to remain at home in your last 3 months of life, with funding of up to $25,000 over 12 weeks.
