Live at home longer where you are more comfortable and surrounded by family with support from Nhulundu Health Service Aged and Community Care Programs.

Nhulundu Health Service has been successfully delivering aged care services for more than 16 years.   Our dedicated and professional team has been assisting the elderly and people living with disabilities to live fulfilling and independent lives in their own homes for longer. These services are cost-effective, culturally appropriate and tailored to meet the patient’s or client’s individual needs.

Am I eligible?

To be eligible for assessment you must meet certain criteria:

  • What assistance do you require with everyday tasks?
  • You must be aged 65 years or older (50 years or older for Aboriginal or Torres Strait Islander people)

If you are on a low income, are homeless or at risk of being homeless, and aged 50 years or older (45 years or older for Aboriginal or Torres Strait Islander people), please call the My Aged Care centre on 1800 200 422 to discuss your situation.

What will it cost?

The Australian Government subsidises a range of aged care services in Australia.  You may be asked to contribute to your service, based on your individual financial circumstances. Each person has different needs and circumstances and therefore, each care plan is specific and cost differently. The funding program or package through which you access aged care services at home will determine how much you will need to pay. Fees and eligibility for the subsidy will be discussed with you before you receive the relevant services.

Our Services

Nhulundu Health Service Aged Care Programs provide three service delivery options to people who are aged, people living with a disability, or their carers. These may take the form of any of the below programs – or a combination of services from each program to meet your specific needs.

  1. Home Care Packages:

This program helps people live independently in their own homes for as long as they can. The Australian Government provides a subsidy to Nhulundu to deliver your package of care.

  1. Commonwealth Home Support Program:

This program is an entry-level home help program for older people who need some help with daily tasks to live independently at home.

  1. Community Transport:

This program is to help eligible individuals who experience transport disadvantages and are:

  • Under 65 years of age; and
  • Unable to access, or have limited access, to other transport options, including public or private transport or transport funded through other funded/subsidised schemes; or
  • An unpaid carer, travelling in their role as a carer.
