Housing Program Officer

 (Part time position based in Gladstone- minimum 2/3 days per week)

Gladstone Region Aboriginal and Islander Community Controlled Health Service (GRAICCHS) trading as Nhulundu Health Service, is an Aboriginal Community Controlled Health Service delivering an integrated, comprehensive primary care health service to the whole Gladstone community.

The Assistance with Care and Housing Sub Program, targets the provision of support to elderly community members who are on a low income and who are homeless or at risk of homelessness as a result of experiencing housing stress or not having secure accommodation

The position of Housing Program Officer will provide practical services and support to ensure that the programs target group of vulnerable elderly people are supported in being housed appropriately and that they are receiving the care they need to continue living in the community.

You will:

  • identify vulnerable elderly community members who qualify for program support
  • establish goal focused client management plans, including support with My aged Care assessment processes
  • identify housing and services providers and options
  • coordinate service responses to ensure appropriate housing and care is secured


Key Requirements include:

  • Experience in Social Work and /or Housing Support/ Community Care program – client planning and services delivery roles – desirable
  • Preferred qualification = Certificate IV in Community Services or Aged Care, or Certificate IV in Service Coordination (ageing and disability); or relevant level of experience – highly desirable.
  • Demonstrated skill and/or ability to communicate effectively and sensitively with Aboriginal and Torres Strait Islander persons and socially isolated and/or disadvantaged persons
  • An understanding of the health and social issues impacting upon indigenous people in the area
  • Understanding of Aged Care service policies, systems and procedures
  • Excellent organisational and problem solving skills
  • Current C Class Drivers Licence and ability to travel especially across the designated area
  • Satisfactory Police Check with no disclosures that preclude you from employment
