Home Care Nurse/Assessor

(Part time position based in Gladstone – 24 hours per week)

 

Gladstone Region Aboriginal and Islander Community Controlled Health Service (GRAICCHS) trading as Nhulundu Health Service, is an Aboriginal Community Controlled Health Service delivering an integrated, comprehensive primary care health service to the whole Gladstone community.

The Aged Care team coordinates and provides in home care and out of home day activity services to eligible clients in the Gladstone Region. The Team works with the local Indigenous communities and relevant key stakeholders to provide a coordinated comprehensive consumer directed care service with a focus on assisting the elderly receive quality in house care and increased access to care and health services as required.

We are looking for dedicated professional to provide care and support to our elderly clients.

  • Base Salary (negotiable dependent on qualifications and experience)
  • 9.5% Superannuation Contribution
  • Salary Sacrifice $15k
  • 5 weeks Annual Leave (pro rata for hours worked)
  • Standard hours = 24 hours week (some flexibility with times/days )

 

You will

  • Deliver in home nursing services to Home Care Package clients in line with Client Contracts and scope of practice, this includes: general nursing, wound care, catheterization, medication support and education, injections
  • Contribute to the ongoing review of existing Home Care Package clients’ needs and support ACAT assessment processes
  • Support the identification  of priority CHSP clients for ACAT assessment and facilitating eligibility and needs assessment processes
  • Contribute to the development of client care plans for new GRAICCHS Home Care Package clients
  • Complete daily/weekly documentation. Recording all instances of planned and unplanned service delivery

 

Key Requirements include

  • Nursing Degree or relevant Medical tertiary qualifications with current Nursing Registration  with AHPRA as a RN and current practising certificate
  • Immunisation Endorsement and experience
  • Demonstrated experience in aged care nursing and/or provision of outreach/in home nursing services
  • Ability to communicate effectively and sensitively with Aboriginal and Torres Strait Islander persons
  • Current C Class Drivers License (Qld)
